- Primary Prevention: Challenging Risk Profile, 2 or more risk factors
- Secondary Prevention: Post MI, Heart Failure, PAD, Cardio/Vascular condition (see referral for complete information)

- Primary Prevention: Stable Hypertension, Stable Diabetes, Dyslipidemia, Metabolic Syndrome
- No established cardiovascular disease

- Positive diagnosis of Type 1 or Type 2 diabetes, Impaired Fasting Glucose, Imparied Glucose Tolerance & Gestational Diabetes
